MENTAL HEALTH SERVICES
Supported Housing Program
In the Supported Housing Program residents live with two other house mates. The participants are self-sufficient for the most part but may receive guidance/assistance to access necessary services. Services may include assistance securing alternative permanent housing, case management, mental health, rehabilitation, respite, social opportunities, medical, and employment supports. The housing sites are furnished and general utilities are included in the participants rent. Rent is calculated at 30% of the participants income for that month. General maintenance/repairs are handled by the agency.
The program is funded through the Office of Mental Health, primarily through community reinvestment.
Treatment Apartment Program
In the Treatment Apartment Program residents live with two other house mates. The participants are generally independent in many areas of daily living skills, but may continue to require minimal support and supervision. Resident Advisors make visits to the sites seven days per week and are available 24 hours a day for emergencies. Residents of the apartments share the housekeeping, shopping and cooking responsibilities. All participants are expected to complete a minimum of 20 hours a week in some form of structured setting such as continuing day treatment program, vocational training, volunteer work, employment or schooling. All needed clinical services are provided on an outpatient basis by local mental health providers. Each participant is involved in creating their own Individual Service Plan with the assistance of the interdisciplinary team to help them to achieve their own goals and address any areas of need to aid them in increasing their level of independence. This plan and the participants' progress and needs are reviewed monthly by the team and quarterly with the participants.
The program is funded by Medicaid and licensed by the Office of Mental Health.
